REGISTER YOUR TEAM BY POSTAL MAIL 1. Print & complete the forms listed below. NOTE: Only the $50, and the CBA Membership Form needs to be sent to CBA. If you are registering for a tournament the same time you are paying for CBA membership, the Tournament Entry Form (see #2 below) will also need to be mailed. All other forms will need to be in your possession and taken to each tournament. CBA officials will check all forms prior to tournament play. MEMBERSHIP FORM (1) PARENTAL CONSENT/PLAYER INSURANCE WAIVER (1 PER PLAYER)
